Affordable Care Act (ACA) 1095-C Printing
We want to keep you up-to-date on PDS ACA reporting and specifically how Vista will support your needs. PDS will be offering the Standard ACA Form, the ACA Mailer and the ACA Pressure Seal options. Each option is described below with a link to a sample form.
The 1095-C forms that Vista HRMS supports for 2017 are listed below.
- Standard 1095-C ACA form – Vista prints the data, form and instructions on a blank 8.5 x 11 sheet of paper. Two sided printing is required. Click here for a sample standard 1095-C ACA form.
- 1095-C ACA Mailer – Vista prints the data, form and instructions on a blank 8.5 x 11 sheet of paper. Two sided printing is required. This form is designed to be used with envelope 9999-1 and 9999-2. This is the same envelope that is used for the Employee W-2 laser. Click here for a sample 1095-C ACA Mailer form.
- 1095-C ACA Pressure Seal – This form will need to be ordered as it is blank on the front with printed instructions on the back and is designed to be used with a pressure seal machine. Click here for a sample 1095-C ACA Pressure Seal form.
